1. Do not claim these were made by you. Credit is required somewhere on your site.

2. Only credit me with buttons provided on my link page. click here

3. Maximum button request is one every 2 weeks. Please don't apply every day.

4. If you are unhappy with your button, I can remake it for you. You will need to fill out this form again.

5. Do not put any button links from piczo zone they must be direct links from either: photobucket or tinypic

6. If you are not too sure how to use this form just ask me before requesting.

7. Dont request any cursors this form is for buttons only.

8. If you don't have a button and need one you may use one of my premade buttons. click here

9. I will send your button to your website or via email when it is made, so please leave either your site url or email when applying.

10. When I send your image please save it on your computer and upload it to your own image host.
